Change of Modality Request

If a student wants to change their modality from in-person to online (or vice versa), they should complete the Change of Modality Request form here:

  1. The student will enter their current modality and their desired modality.
  2. Upon making selections for the two modality fields, a paragraph is presented to which the student must agree with an electronic signature and the date.
  3. The student can see but not enter data into the fields for approval information that will need to be completed by an administrator.
  4. Upon submitting the form, a confirmation email will be sent to the student with their advisor and spsadvising@columbia.edu CCed.


The program administrator will consult with an administrator for the student's program of study. When a decision is made, the Advising administrator will enter data in the approval fields. The administrator is responsible for providing the approval information to the student.
