FERPA Release Form

If a student wants to authorize an individual or organization that can speak on their behalf, they should complete the FERPA release form:

  1. The student enters the following information about the authorized person/organization:
    1. Name
    2. Mailing address
    3. Email address
    4. Phone number
  2. The student provides an electronic signature and date based on a paragraph granting the authority.
  3. Upon submitting the form, a confirmation email goes to the student and to spsadvising@columbia.edu

FERPA release information is displayed on students' main profile pages in Slate on a tab called "FERPA Releases."

On that tab, there are separate sections for "Active" and "Inactive" FERPA releases. (FERPA releases should not be deleted. If a person/organization is no longer authorized, the status should be set to "Inactive."

To modify FERPA release information, the Advising team member, should click on the release from the "FERPA Release" tab. A form will pup up in which they can edit the mailing address, email address, or phone number of the individual/organization. They can also change a FERPA release from active to inactive or vice versa.
