Change of Modality Request
If a student wants to change their modality from in-person to online (or vice versa), they should complete the Change of Modality Request form here:
https://advising.sps.columbia.edu/register/change_of_modality_form
- The student will enter their current modality and their desired modality.
- Upon making selections for the two modality fields, a paragraph is presented to which the student must agree with an electronic signature and the date.
- The student can see but not enter data into the fields for approval information that will need to be completed by an administrator.
- Upon submitting the form, a confirmation email will be sent to the student with their advisor and spsadvising@columbia.edu CCed.
The program administrator will consult with an administrator for the student's program of study. When a decision is made, the Advising administrator will enter data in the approval fields. The administrator is responsible for providing the approval information to the student.
