Student/Staff Duplicate Form Submissions

Test Forms 797/798

  • Identify/confirm duplicate form submissions.

  • If duplicate, retain earliest form submitted (e.g. 797).

  • On Later/est Form(s) Submitted (e.g 798):

    • Update keyword value “Prior Form Number: to the number for the earliest form submitted (e.g. 797).

    • Add note to History tab explaining why the form is being stopped; for example:
      Form stopped; duplicate submission of form number 797.

    • Stop Form.

Staff Revision Form Submissions

Test Forms 791/808

  • Review/confirm no discrepancies/issues exist (example below).

  • If none exist, retain latest form submitted (e.g. 808)

  • On earlier/est Form(s) submitted (e.g. 791)

    • Add note to History tab explaining why the form is being stopped; for example:
      Form stopped; superseded by form number 808.

    • Stop Form.

Submitted Early & Not Registered for Courses in a Term/sub-term Before the Term of Withdrawal

(Test Forms 752, 790)

Includes subsequent need to move a form out of the Future Form Hold Queue than intiially expected.

  • Part 1: Submitted Early

    • Select “Place on Hold.”

    • Enter the date the form should resume processing; this will be the new Withdrawal Effective Date and Date of Determination.

  • Part 2: Return to Processing early - on the Current/a Previous Date

    • Navigate to “Future Attendance Form Hold” queue and select the applicable form.

    • Select “Return for Processing (Bypass Hold Until Date)”; the date this occurs will be the new Withdrawal Effective Date and Date of Determiantion.

      • If these dates are not appropriate as a result of the information requiring the form to be placed back in processing, please email universitywithdrawals@columbia.edu for assistance with a manual adjustment.

    • Part 3: Return to Processing Early - on a Revised Future Date.

      • Navigate to “Future Attendance Form Hold” queue and select the applicable form.

      • Select “Return for Processing (Bypass Hold Until Date)”.

      • Select the applicable form from within the queue where it now resides (e.g. RO).

      • Select “Place on Hold.”

      • Enter the date the form should resume processing; this will be the new-new Withdrawal Effective Date and Date of Determination.

Submitted Early & Registered in a Future Term
