A Systems Access Request form is required to obtain a GSB email account.
Complete this form and send to ITG <admsup@gsb.columbia.edu>View file name CBS Systems Access Authorization Form.pdf 
You must indicate a “Last Date of Employment” for all contractors. This date cannot exceed one year from the start date.
